New entry with basic information from the disc and Amazon (for DVD release date) just to get this in the databsae for others to build from.

This not a blu-ray/dvd combo So I unchecked dvd

comes in hd slim with slip cover

video format automatically changed when unchecked dvd

front and rear covers
