1) updated roles and order to match closing credits, including the dividers which are in the credits;
2) Used "As Credited" for Matt Ross, since Matt appears to be his working name since the mid 90's (Face/Off, Big Love, for example)
3) Changed Ildiko Jaid & la Valette's names to match credits and since credited names appear to be common i didn't 'Credit As' them.
4) Dropped DeFinna from Clubgoers as he isn't listed in the credits

Cover Scans:  Front: a little cleaner, better edges; back: less noise in the overview text, all the pink text is now legible.

Produced: to 1998.
Released May 1998 (http://www.imdb.com/title/tt0120728/releaseinfo).  Review release dates match that (http://rogerebert.suntimes.com/apps/pbcs.dll/article?AID=/19980529/REVIEWS/805290302/1023).  I saw it in theatrical release that same week per my records.
