If anyone cares, the rating for the r cut on disc also are
"Accident Gore, Some Graphic Language and Aberrant Sexual Content"

CoO from end credits.
Alliance is the copyright holder in end credits, Fine Line is US but was just the distributor.

Full crew from open and end credits of film.
"Written by" is the writing credit.
J. G. Ballard credited as J G. 3/34 to 1/2 to 2/5 (J.G., none for JG)
BY for David Lee from approved BY list, and looks to be correct one from all link's crecits.
