It has been pointed out to me that the rules do not specifically address this issue and might use the Title rule as well.

Ken has a post in the Men in Black thread I believe can be applied here: http://www.invelos.com/Forums.aspx?task=viewtopic&topicID=667247&messageID=2082773.

Since my experience has been that we copy data exactly as written, unless specifically outlined in the rules, I am letting this contribution stand.

Based on the findings in this thread, Mr. Mouat's common name is the variant *without* the middle initial. As such, he can simply be listed "as credited" here.
