What was the rule for episode titles (i.e. source)?

The episode title in question is "Dreimal schwarzer Kater" versus "3 x schwarzer Kater".

And yet this is the actual onscreen title:



The rule states

Take the episode title from the episode itself, or from elsewhere on the DVD or DVD packaging, exactly as shown.


But does this imply an order of preference, or not?

Always take the title from the episode itself if you are fortunate enough to find a title at the beginning.

Count me in as giving precedence to the episode titles seen on-screen - if there are any, of course. It's interesting to note, however, that while everyone seems to agree, this same principle is often ignored in German profiles for non-German TV shows, where users substitute original-language episodes titles seen on-screen in favour of episode titles translated into German - resulting in the fact that the episodes' original titles (which are much more important to me than the translated ones) aren't mentioned anywhere in the profile. I feel the episode titles seen on-screen should take precedence there as well.

It's fine to list the translated titles in the overview field, but there can be no excuse for completely leaving out the actual, original episode titles out of a profile when they're actually seen on-screen.

It's not only ignored in German contribution, but also in many other territories with a dubbing culture.
I own some French series boxsets of some US shows and the episode titles in those profiles are (not surprisingly) in French.

I think this is also the most useful way: Episode titles matching the profile's locality.

BTW:
The on-screen episode title mostly is in English but I came across several German releases where the German episode title is displayed via subtitles when the original title is being shown.
And I don't mean that you have to chose a subtitle track before playing the episode but it appears automatically when playing those disc on a DVD/BD player with German menu settings.
So in those cases both titles appear on-screen.
