Added Overview from the cover. Used ' around the italicised parts as per the current rules.

NOTE:
I have added dividers into the Art crew list to make it more relevant. Each of these entries does have the effects house name as a header before the people are credited...so I see no reason for this to not be allowed (this is the very same principal employed with dividers in cast lists). Also, the rules do not state that effects companies cannot be listed in dividers.

ADDED:
3.5 crew from credits in accordance with contribution rules.
