Hi,
I just did a profile refresh and noted that there were format changes indicating that three dvds were not anamorphic when they were.  Is there something going on that I'm not aware of?  The only name that I recall was "Survivor".
Also, I understand the requirements for birth years but what type of proof is required to have one removed?

This question has to be split up into two parts:
a) How to document?
Either you document, that the BY is wrong and assign another one (here a recent example) or you document that there are only entries for one person with the name (often after a common name was established or changed as seen here). Any invalidation should be documented in this thread.
b) How to get it out of the online data base?
Open a support ticket. At least in the past this worked.
Complete list of Common Names  •  A good point for starting with Headshots (and v11.1)
