I just finished the credits for the Dutch 2-disc edition (5-050582-323177), and I wanted to share this info with other localities.

Updated crew, from end credits unless noted otherwise:

Cast common names, found with the CLT:
James Ritz (110/342), Jim Ritz (11/75)
Jane Jenkins (58/195), J. J. Chaback (20/63). Ms. Jenkins is usually a casting director, but uses this alias when she appears in a movie: http://www.filmreference.com/film/34/Jane-Jenkins.html

When checked with the CLT, this common name is wrong:
Ben Bodé: 38/77
Ben Bode: 42/123, and 27 are incorrect Apollo 13 credits. When these are corrected, the score will be 104 vs 96 profiles in favour of Ben Bodé. Same goes for Michelle Little.
