Walter P. Martishius [Walter Paul Martishius] - CLT: 58 titles vs. 4 titles.

Also, Richard Cohen is credited as "Digital Supervisor", which, per the crew credits table in the contribution rules, does not qualify for entry. "Digital Effects Supervisor" would qualify, "Digital Supervisor" does not. See this recent forum poll for reference:
